Economic Burden of Psoriasis
Psoriasis may cause a substantial economic burden to patients, health service providers, third parties, and the society as a whole. However, all of these costs may not be adequately considered when assessing the treatment costs for psoriasis. Psoriasis may negatively affect work productivity as psoriasis has a relatively high incidence in working age people that lead to possible costs because of lost productivity.
The aims of this thesis were to estimate the economic burden of psoriasis particularly from patients’ and health service providers’ perspectives and to estimate the background factors (e.g., severity of psoriasis) that may have led to high costs. Another aim was to estimate the total medication costs and to estimate psoriasis’ proportion of health-related productivity losses.
The patient sample was based on patients with psoriasis who visited the Department of Dermatology in Turku University Hospital during a one-year study period. These patients were sent a questionnaire. From the patients who gave consent, medication information, clinical information, and number of visits to Turku University Hospital were collected. This data was linked to the information from the questionnaire.
Overall psoriasis was estimated to cause a substantial economic burden for the patient, health service provider, health insurance system, employer, and the society as a whole. The direct costs represented only a small proportion of the overall financial burden of psoriasis, whereas indirect costs were significant. The estimated annual costs for patients and employers were almost twice the costs to health service providers or the Social Insurance Institution of Finland.
In conclusion, the cost contribution of patients and employers should be considered when assessing the costs of different treatments, in addition to commonly studied direct costs of medications and costs to health service providers. Methods used to assess these costs should be well justified and be described clearly to allow comparisons between studies and to evaluate the quality of the results.Psoriaasi saattaa aiheuttaa merkittävän taloudellisen taakan potilaille, sairaalapalvelujen tuottajalle, kolmansille osapuolille ja yhteiskunnalle. Näitä kuluja ei kuitenkaan ole otettu aina huomioon, kun psoriaasin kustannuksia on arvioitu aiemmissa tutkimuksissa. Psoriaasi saattaa vaikuttaa negatiivisesti potilaan työkykyyn, sillä psoriaasilla on korkeahko esiintyvyys työikäisissä ihmisissä, aiheuttaen mahdollisesti tuottavuuden madaltumista. Tutkimuksen tavoitteena oli selvittää psoriaasin taloudellinen kokonaistaakkaa erityisesti potilaiden ja sairaalapalvelujen tuottajan näkökulmasta ja arvioida mahdollisten taustatekijöiden (esim. psoriaasin vaikeusaste) vaikutusta kustannuksiin. Tavoitteena oli myös lääkityksen kokonaiskustannusten arviointi ja arvioida psoriaasin aiheuttamaa osuutta terveyteen liittyvästä työkyvyn menetyksestä.
Tutkimusten aineistoksi valittiin psoriaasia sairastavat potilaat, jotka kävivät Turun yliopistollisessa keskussairaalassa ihotautiklinikalla vuoden tutkimusjakson aikana. Näille potilaille lähetettiin kyselylomake. Suostumuksen antaneilta potilaita kerättiin tiedot käytetyistä lääkkeistä, kliinisiä tietoja ja käynnit Turun yliopistollisessa keskussairaalassa. Nämä tiedot yhdistettiin kyselylomakkeen tietoihin.
Psoriaasin arvioitiin aiheuttavan merkittävän taloudellisen taakan potilaalle, sairaalapalvelujen tuottajalle, sairausvakuutukselle, työnantajalle ja yhteiskunnalle. Suorat kustannukset muodostivat pienen osan kokonaiskustannuksista epäsuorien kustannusten ollessa merkittäviä. Psoriaasin aiheuttamat vuosittaiset kustannukset potilaille ja työnantajille olivat lähes kaksinkertaiset verrattuna sen aiheuttamiin kustannuksiin Kansaneläkelaitokselle tai sairaalanpalvelujen tuottajalle.
Johtopäätöksenä totean, että psoriaasin aiheuttama taakka potilaille ja työnantajille tulisi ottaa huomioon, kun arvioidaan psoriaasin hoitomuotojen kustannuksia ja seurauksia, tyypillisesti huomioitujen suorien lääkityskustannusten ja sairaalapalveluiden tuottajille koituvien kustannusten lisäksi. Näiden kustannusten määrittelemiseen käytettyjen menetelmien tulisi olla hyvin perusteltuja ja selkeästi kuvailtuja, jotta tuloksia voidaan vertailla ja niiden laatua arvioida.Siirretty Doriast
Keskitetty lokienhallinta HP ArcSightilla
Tämän työn tavoitteena oli tutkia työn tilaajalle Ymon Oy:lle sitä, kuinka Hewlett-Packardin ArcSight-tuoteperheen avulla rakennetaan keskitetty lokienhallintajärjestelmä ja miten hyvän lokienhallinnan tavoitteet toteutuvat.
Tietoturvan osa-alueita ovat luottamuksellisuus, eheys, kiistämättömyys ja saatavuus. Lokienhallinta liittyy olen-naisena osana kaikkiin näihin neljään osa-alueeseen ja parantaa niiden toteutumista valitussa ympäristössä. Lähes poikkeuksetta kaikki tietoliikenneverkossa toimivat laitteet tuottavat lokia. Lokitiedon avulla saadaan tarkka käsitys tapahtumasta, ajanhetkestä ja autentikointia käytettäessä tekijästä. Keräämällä lokitiedot keskitettyyn lokienhallintajärjestelmään ja normalisoimalla eri lokit yhtenäiseen lokimuotoon saadaan kokonaiskuva koko järjestelmän tapahtumiin. Keskitetystä järjestelmästä voidaan vaivattomasti nähdä tarvittavat tiedot helposti verrattuna hajallaan oleviin eri lokiformaatteja sisältäviin järjestelmiin. Nämä vaatimukset ovat erityisen tärkeitä luottamuksellisia tietoja käsittelevillä organisaatioilla, kuten luottokorttiyhtiöillä.
Tämän työn tuloksena syntyi tutkielma, jossa arvioidaan, miten ohjeistukset ja säännökset erityisesti
VAHTI-lokiohjeessa vaikuttavat lokienhallinnan toteutukseen. Lopputuloksena syntyi myös pelkistetty malli miten yksinkertainen ja vikasietoinen keskitetty lokienhallintajärjestelmä voidaan toteuttaa HP ArcSightin lokienhallintatuotteilla.The purpose of this Bachelor’s thesis was to study a centralized log management system for Ymon Oy. This study concentrates on examining the centralized log management system, how it is implemented with Hewlett Packard ArcSight and how well the log management objectives are met.
The aspects of information security include confidentiality, integrity, non-repudiation and availability. Log management is an essential component of each of these four areas and significantly improves their implementation. Almost without exception all the communication network devices produce log information. Log information tells the exact nature and time of the event and also of its author, if authentication is used. Gathering log information into a centralized log management system and normalizing it to one log format gives an overview of the events of the whole system. Without this kind of system log information must be collected from various scattered devices and systems. Managing log information efficiently and securely is often a necessary requirement for organizations that handle confidential information.
This thesis resulted in a study which assesses how the guidelines and regulations, regarding the VAHTI log-guide in particular, affect the implementation of the log management system. This work also includes the basics of planning, designing and implementation that need to be considered when creating a simple and fault-tolerant centralized log management system with HP ArcSight products. The thesis is especially helpful for those who are looking for information about what kinds of problems may appear at various stages of the development when building a log management system
The Delay and Costs of Diagnosing Systemic Vasculitis in a Tertiary-Level Clinic
Introduction The diagnosis of systemic vasculitis is a challenge because of the heterogeneity of clinical manifestations. The aim of this study is to analyze the diagnostic delay in systemic vasculitis, the total costs during the first year of care, and how the diagnostic delay affects the costs in a tertiary health care facility. Methods Patients with a new diagnosis of systemic vasculitis between 2010 and 2018 were identified from hospital records. The diagnostic delay and health care costs were evaluated during the diagnostic period and within 12 months after the first contact with tertiary health care. Vasculitis-related costs were recorded as true costs charged. A total of 317 patients fulfilled the study criteria. The diagnoses were grouped into three clinically relevant groups: IgA vasculitis and other small-vessel vasculitis (n = 64), ANCA-associated vasculitis (AAV) (n = 112), and large-vessel vasculitis (LVV) (n = 141). Results The diagnostic delay from the first referral to tertiary-level clinic was shortest in the LVV group and longest in the AAV group. Total costs during the diagnostic period were the highest in the AAV group (median = €6754 [IQR €8812]) and lowest in the LVV group (median = €3123 [IQR €4517]), p s = 0.38, p s = 0.34, p p Conclusions There is a substantial diagnostic delay that correlates significantly with the costs in tertiary-level health care when diagnosing systemic vasculitis.</div
Inflammatory bowel disease-related economic costs due to presenteeism and absenteeism
Background: Inflammatory bowel disease (IBD), consisting of Crohn's disease and ulcerative colitis, can be a lifelong burden generating high costs to an economic system. Data regarding the cost of workplace presenteeism and absenteeism in workers with IBD are limited. Our objective was to assess these costs in employed adults.Methods: A structured questionnaire, hospital records and national registers were combined to assess the economic costs involved with workplace presenteeism and absenteeism in employed patients. Our final sample comprised 320 IBD patients. The costs were calculated as productivity-loss costs by using a Human Capital Approach.Results: Due to IBD, the mean annual economic costs of workplace presenteeism were euro643.90/patient, and mean annual absenteeism costs were euro740.90/patient. Women had higher costs (euro955/patient/year) from absenteeism compared to men (euro531/patient/year) especially when working blue-collar jobs. These findings were also evident in presenteeism. CD and UC patients had similar total costs due to presenteeism and absenteeism. The use of biologics did not have a major impact on these costs.Conclusion: IBD patients had moderate economic costs from workplace presenteeism and absenteeism. Interestingly, women, working blue-collar jobs, had higher costs than men.</div
Inflammatory bowel disease: perceived impact on leisure-time activities
Objective: Inflammatory bowel disease, consisting of Crohn's disease and ulcerative colitis, is known to negatively impact an individual's quality of life. Leisure-time activities are an important part of life by creating and strengthening social networks. The aim of this study was to thoroughly evaluate the perceived impact of inflammatory bowel disease on leisure-time activities by quantifying limitations in activities caused by the disease.Methods: A structured questionnaire, hospital records and national registers were combined to assess limitations caused by the disease in a patient's leisure-time activities. The final study sample was 561 patients.Results: More than half of the patients (52.8%) reported that IBD had caused limitations in their leisure-time activities. Women perceived that their limitations were greater when compared to the reports by men. One-third of the patients (33.3%) reported reducing and 17.6% abandoning at least one leisure-time activity due to their disease. Most often mentioned activities that were reduced and abandoned were physical activities. The IBDQ32 score had a significant correlation with all of the studied outcomes. Laboratory tests results and the patient's age or the level of income did not correlate with limitations in leisure-time activities.Conclusions: In conclusion, this study showed that patients with inflammatory bowel disease perceived a marked limitation in their leisure-time activities due to their disease.</div
Five Baltic Ports Togehter: Forecasts, Trends and Recommendations
The ports of Stockholm, Tallinn, Helsinki, Naantali and Turku play key roles in making
the Central Baltic region accessible. Effective, competitive, eco-friendly and safe port
procedures and solutions for the transportation of goods are of major importance for
trade in the Baltic Sea region. This report presents the most essential results and
recommendations of the PENTA project, which focused on how ports could better
comprehend and face current and future challenges facing carriage of goods by sea.
Each of the four work packages (WPs) of the PENTA project analysed the changes from
a different perspective. WP2 focused on traffic flows between the PENTA ports. Its
main emphasis was on the ports, shipowners, and logistics companies that are the key
parties in freight transport and on the changes affecting the economy of those ports. In
WP3 noise as an environmental challenge for ports was investigated and the analysis
also shed light on the relationship between the port and the city. In WP4 procedures
related to safety, security and administrative procedures were researched. The main
emphasis was on identifying the requirements for the harmonisation of those
procedures.
Collaboration is highlighted throughout this report. In order to prepare for the future, it
was found that ports need to respond to growing competition, increasing costs and shifts
in customer demand by strengthening their existing partnerships with other actors in the
maritime cluster. Cargo and passenger transport are the main sources of income for
most ports. Cargo traffic between the PENTA ports is expected to grow steadily in the
future and the outlook for passenger traffic is positive. However, to prepare for the
future, ports should not only secure the core activities which generate revenue but also
seek alternative ways to make profit. In order to gain more transit traffic, it is suggested
that ports conduct a more thorough study of the future requirements for doing business
with Russia.
The investigation of noise at ports revealed two specific dilemmas that ports cannot
solve alone. Firstly, the noise made by vessels and, secondly, the relationship between
the port and the surrounding city. Vessels are the most important single noise source in
the PENTA ports and also one of the hardest noise sources to handle. Nevertheless, port
authorities in Finland and Sweden are held responsible for all noise in the port area,
including noise produced by vessels, which is noise the port authority can only
influence indirectly. Building housing by waterfront areas close to ports may also
initiate disagreements because inhabitants may want quiet areas, whereas port activities
always produce some noise from their traffic. The qualitative aspects of the noise
question, cooperating with the stakeholders and the communicating of issues related to
noise are just as important. We propose that ports should follow the logic of continuous
improvement in their noise management.
The administrative barriers discussed in this report are mainly caused by differences in
international and national legislation, variations in the customs procedures of each
country, the incompatibility of the IT systems used in maritime transport, noncompliance
with regulations regarding dangerous goods, and difficulties in applying
Schengen regulations to vessels from non-EU countries. Improving the situation is out
of the hands of the ports to do alone and requires joint action on a variety of levels,
including the EU, national authorities and across administrative borders.Tukholman, Tallinnan, Helsingin, Naantalin ja Turun satamat ovat merkittävässä

Rahtiliikenteelle suunnitellut tehokkaat, kilpailukykyiset, ympäristöystävälliset ja
turvalliset satamatoiminnot sekä -ratkaisut ovat tärkeä osa Itämeren alueen kauppaa.
Tämä raportti esittelee PENTA-hankkeen tärkeimmät tulokset ja suositukset, jossa
selvitettiin miten satamat voisivat paremmin ymmärtää ja valmistautua nykypäivän sekä
tulevaisuuden meriliikenteen haasteisiin.
PENTA-hankkeen neljästä työpaketista jokainen analysoi tulevia muutoksia eri
näkökulmasta. Toinen työpaketti keskittyi liikennevirtoihin PENTA-satamien välillä.
Painotus kohdistui satamiin, varustamoihin ja logistiikkayrityksiin rahtiliikenteen
merkittävimpinä osapuolina, sekä muutoksiin jotka vaikuttavat yleiseen
taloustilanteeseen. Kolmannessa työpaketissa käsiteltiin melua satamien
ympäristöhaasteena sekä luotiin tämän pohjalta katsaus satamien ja kaupunkien välisille
suhteille. Neljännessä työpaketissa tutkittiin turvallisuuteen, turvatoimiin, sekä
hallinnollisiin toimenpiteisiin liittyviä seikkoja. Keskeisin tavoite oli osoittaa tarve
menettelytapojen harmonisoinnille.
Yhteistyön merkitystä korostetaan raportissa kautta linjan. Jotta satamat pystyisivät
valmistautumaan tulevaisuudenhaasteisiin, tulee näiden pystyä vastaamaan kasvavaan
kilpailuun, nouseviin kustannuksiin ja muutoksiin kuluttajien kysynnässä vahvistamalla
voimassa olevia kumppanuuksiaan meriklusterin sisällä. Rahti- ja matkustajaliikenteestä
aiheutuvat tulot ovat pääasiallinen tulonlähde useimmille satamista. PENTA-satamien
rahtiliikenteen odotetaan kasvavan tasaisesti tulevaisuudessa ja näkymät
matkustajaliikenteelle ovat myös positiiviset. Valmistauduttaessa tulevaisuuteen ei tulisi
ainoastaan keskittyä nykyisiin ydintoimintoihin vaan myös etsiä uusia tulonlähteitä.
Kauttakulkuliikenteen kasvattamiseksi on suositeltavaa, että satamat tekevät
perusteellisemman tutkimuksen Venäjän liiketoiminnan tulevaisuuden vaatimuksista.
Satamamelun tutkiminen paljasti kaksi dilemmaa, joita satamat eivät pysty yksin
ratkaisemaan. Ensimmäinen ongelma on aluksista lähtevä melu ja toinen ongelma on
sataman ja sitä ympäröivän kaupungin välinen suhde. Laivat ovat suurin yksittäinen
melunlähde PENTA-satamissa ja samalla yksi vaikeimmin pienennettävissä olevista.
Siitä huolimatta satamaviranomaisia pidetään vastuullisina kaikesta satama-alueella
syntyvästä melusta Suomessa ja Ruotsissa – mukaan lukien alusmelu johon satamat
eivät suoraan voi vaikuttaa. Asutuksen rakentaminen meren ääreen ja satamien lähelle
aiheuttaa myös erimielisyyksiä, sillä asukkaat kaipaavat usein hiljaisuutta ja
satamatoiminnot aiheuttavat aina jotain melua. Melukysymyksen laadulliset
näkökulmat, yhteistyö eri toimijoiden kanssa ja meluasioista keskusteleminen ovat
kaikki tärkeitä asioita. Raportin suositus on, että satamien tulisi tehdä jatkuvia pieniä
parannuksia melun hallinnassa.
Raportissa käsiteltävät hallinnolliset hidasteet johtuvat pääasiassa maiden välisistä
laintulkinnoista ja niiden eroavaisuuksista, tullimenettelyistä, merenkulkualalla
käytettävien IT-järjestelmien yhteensopimattomuudesta ja muista sääntöjen
tulkintaeroista. Satamat eivät pysty yksin parantamaan tilannetta vaan se vaatii yhteisiä
ponnisteluja Euroopan Unionissa, kansallisella tasolla sekä eri hallinnollisten

Cost-minimization modeling of venous thromboembolism diagnostics: performing limited compression ultrasound in primary health care reduces costs compared to referring patients to a hospital
Background: The aim of this retrospective study was to determine whether diagnosing a deep venous thrombosis (DVT) in primary health care using limited compression ultrasound (LCUS) can save resources compared to referring these patients to hospital. According to the current literature, LCUS is as safe as a standard protocol based on a whole-leg ultrasound (US).Methods: We created a standardized patient for this cost-analysis model based on 76 patients that were referred to hospital for a suspected DVT. Travel distance to the health care centre and hospital was calculated based on the home address. Hospital costs were acquired from the hospital price list and Finnish legislation. Time spent in the hospital was retrieved from hospital statistics. Time spent in the health care centre and travelling were estimated and monetized based on average salary. The cost of participating physicians attending a US training course was estimated based on the national average salary of a general practitioner as well as the course participation fee. A cost-minimization modeling was performed for this standardized patient comparing the total costs, including private and public costs, of standard and LCUS strategies.Results: The total costs per patient of standard and LCUS pathways were 1151.52€ and 301.94€ [difference 849.59€ (95% CI 800.21€-898.97€, p Conclusion: Using LCUS in diagnosing DVT in primary health care instead of referring these patients to the hospital is shown to save a significant amount of public and private resources.</p
Siun soten tiimimalli paransi hoitoon pääsyä
Vertaisarvioitu.Lähtökohdat Siun soten terveysasemilla asiakas odotti vastaanottoa aiemmin jopa kuusi viikkoa. Terveysasemia päätettiin vuonna 2019 kehittää Siun soten tiimimallilla perusteina terveyshyötymalli ja lean ja tavoitteina parempi asiakas- ja henkilöstökokemus, vaikuttavuus ja tuottavuus. Tutkimus valottaa tiimimallin vaikutuksia suomalaisessa terveysasemaympäristössä. Menetelmät Arvioimme asiakaskokemusta, hoidon saatavuutta, saavutettavuutta, jatkuvuutta, turvallisuutta ja henkilöstökokemusta Kiteen, Liperin ja Niinivaaran terveysasemilla keväällä 2021. Tulokset Helmikuussa 2021 Kiteen, Liperin ja Niinivaaran asiakkaista 89–92 % pääsi hoitoon alle viikossa (aiemmin 25–31 %, p < 0,05), ja aika kolmanteen vapaaseen aikaan oli 0 päivää. Kuukaudessa hoidettiin 334–1 050 asiakasta enemmän kuin ennen mallin aloitusta. Niiden osuus, jotka eivät saaneet yhteyttä terveysasemaan, on pienentynyt 10–20 prosentista kahdeksaan prosenttiin. Kiteen hoitajien sairaspoissaolot lisääntyivät. Päätelmät Hoitoon pääsy on parantunut. Asiakastyytyväisyys on hyvällä ja henkilöstön työtyytyväisyys tyydyttävällä tasolla. Pandemian kuormitus vaikeuttaa tulkintaa. Tiimimalli vaikuttaa lupaavalta tavalta vastata perusterveydenhuollon haasteisiin. Subarctic catchment water storage and carbon cycling – Leading the way for future studies using integrated datasets at Pallas, Finland
Subarctic ecohydrological processes are changing rapidly, but detailed and integrated ecohydrological investigations are not as widespread as necessary. We introduce an integrated research catchment site (Pallas) for atmosphere, ecosystems, and ecohydrology studies in subarctic conditions in Finland that can be used for a new set of comparative catchment investigations. The Pallas site provides unique observational data and high-intensity field measurement datasets over long periods. The infrastructure for atmosphere- to landscape-scale research in ecosystem processes in a subarctic landscape has recently been complemented with detailed ecohydrological measurements. We identify three dominant processes in subarctic ecohydrology: (a) strong seasonality drives ecohydrological regimes, (b) limited dynamic storage causes rapid stream response to water inputs (snowmelt and intensive storms), and (c) hydrological state of the system regulates catchment-scale dissolved carbon dynamics and greenhouse (GHG) fluxes. Surface water and groundwater interactions play an important role in regulating catchment-scale carbon balances and ecosystem respiration within subarctic peatlands, particularly their spatial variability in the landscape. Based on our observations from Pallas, we highlight key research gaps in subarctic ecohydrology and propose several ways forward. We also demonstrate that the Pallas catchment meets the need for sustaining and pushing the boundaries of critical long-term integrated ecohydrological research in high-latitude environments.Peer reviewe
